All new patients must complete forms before treatment can begin. I provide the necessary documents below. Please return the completed documents to me either in person or as a PDF file via email before your initial visit.

At your first office visit, we begin with a one-on-one about your case. Before beginning treatment, we want to be certain you are a good candidate for treatment at the clinic, and that you are familiar and comfortable with my philosophy and program.

During this first intake, I will go over your case and medical history in detail, develop a comprehensive treatment plan, and administer your first acupuncture treatment.
